Design Docoment help

Hi All, I know this topic has been touched upon briefly before but I'd like to leverage hobbit at a client site and they require a formal monitoring design document. I was wondering if anybody has created something already. I'd like to put one together and share it with the rest of the group but it would be great if anybody could send me any useful information(diagrams, flow charts, etc).
