Is there any one tried forward/integrate xymon alert to pagerduty?

I've not tried pager duty but I have done Slack.  If PagerDuty has a webhook or other web interface, it should be easy enough to create a script that calls it.  You may want to start here:

Thanks much Johnson. Yes PD has web hook. Do mind if you give me some insight what you did on xymon side, what the script do and where did you put the script.

Thanks Wilfrid, definitely I will try email, the only reason looking other method is PD email integration is the least recommended, but if that work for you, I will try it, I was looking other xymon users feedback.

Just look at the xymon help for alerts.  You can either use a script or
email to manage them when they trigger.  You can use whatever language you
are most comfortable using.  You can also look at the alerts tab on
Xymonton to see some examples:

Are  you asking do we send pages to the person oncall? Yes all the time. We used majordomo to create a mailing list that was the person oncall, both email and text.

We’ve moved on from that to a corporately supported system, but the theory is the same.

Or are you asking for xymon to notify the person of his/her upcoming oncall? Haven’t done that, but it wouldn’t be too hard. Personally, we handled it outside of xymon with some scripts in a cron job.

A couple of departments at the university were testing alternatives to the
old pager process and they tested pagerduty but instead decided to go with
victorops.

I set alerts to both and everything seemed to work as it should.  Victorops
uses much more complicated email addresses but seem to work just fine...for
example:
